If this is your first visit to Paul McLeod’s Australian Acupuncture Centre you will be required to fill out a new client registration form. This form asks for some basic information about you and the reason/s for your consultation. You can fill this new client registration form out upon arrival at the clinic or submit an electronic version of it now by visiting our new client registration page.
